Individual plan
One time enrollment fee: $120
0 - 17 years old: $ 75 per month
18 - 64 years old: $85 per month
> 65 years old: $120 per month
Homebound: $175/hour
Re-enrollment fee: $200​
Late payment fee: $25
Business/Employer Plans
Employer pays full or partial monthly membership.
One time enrollment fee: $100 per employee/dependent.

< 64 years old - $160 per month
> 65 years old - $190 per month
Family plan
2 adults and up to 3 children age less than 17 years old - $ 220
